New re-branded Application for Life and CI Insurance (Form 350)
We are excited to announce that as of December 7^th, 2024, our new re-branded Application for Life and CI Insurance (Form 350) is available on EquiNet^® Individual Insurance>Forms.

We will continue to accept the previous version of the Application for Life and CI Insurance (Form 350) dated May 2023.

Once your current supply is depleted, you can order the new application version dated November 2024 from our Supply department using form 1390 Supply Order Form (Life and Health).
Celebrating Lunar New Year!

Lunar New Year falls on the first new moon of the lunar calendar, January 29, 2025, and celebrations last for about two weeks.

To recognize and celebrate Lunar New Year and Chinese-Canadian culture, we have commissioned a series of works from B.C. artist Steffi Lai. Her piece “Year of the Snake” is featured on our Chinese Markets webpage and other marketing assets. We are thrilled to collaborate with Steffi and showcase her beautiful artwork.

Commission schedule update We’re pleased to share that we’ve released a new and improved Commission Schedule (Schedule “A”), effective October 27, 2025.

What’ s new
- The Commission Schedule (Schedule “A”) now includes all commission details, including FundSERV commissions.
- The “FundSERV – Producer Commission Schedule (Schedule B)” document has been discontinued.
- The schedule has been redesigned to offer greater clarity through plain language and improved structure.
What hasn’t changed
- There are no changes to any commission rates or provisions.
